What is wrong with my battery?

So I plugged my charger into the outlet. Saw the light on the AC adapter brick thing turn green. Then I plugged the charger into my board and saw a spark come from the boards charging port. Then a proof of smoke came out of my AC adapter brick thing and it started flashing red.

Thought maybe it was a bad charger so I bought a new one and the same exact thing just happened again.
What could be going wrong? It’s gotta have something to do with the battery.

I used different outlets when using both chargers. Both times I had the battery disconnected from the ESC.

    Battery details:
    Nominal Voltage - 36V
    Charge Voltage - 42V
    Power - 180Wh
    Nominal Capacity - 5000mAh

    Charger details:
    Input - 100V~240VAC
    50/60Hz 2.0A Max
    Output - DC42.0V - 2.0A

Any help would be appreciated. Thanks!

Jenkins - is the battery outside of the board or do you have the board open and the ESC disconnected from the BMS?
